Forms & documents

These forms are for submitting paper claims, applying for disability benefits, changing your banking information, updating an address, adding/changing a dependent, and assigning a beneficiary for your life insurance, among others.

You can submit your forms via email or fax, but they must have a wet (ink) signature to be processed. Electronic or digital signatures are not accepted at this time.

Accessing your benefits

As a retired member of the NCBAA Trust Fund, you will be able to access your benefits by submitting claims for medical expenses incurred.

You will be able to submit health claims electronically by logging into Sun Life; however, some providers have the option to direct bill.
